Register for Football/Cheerleading

Registration for the 2009 season is easy. If you have access to a printer, we ask you to please download and print out the forms attached to this page. If you do not have access to a printer simply go to our Request Our Registration Packet page and you can fill out your information, and we will send you a packet in the mail. Please fill out every page of the packet completely with a $25 check made out to Downtown Ministries and mail it back to us by July 15th, 2009 if you want your child to participate. Return to:
